Posted August 1, 2025, The Ohio Department of Insurance sent out a reminder that all Business Entities must review and update their affiliations in advance. Affiliations can be reviewed and updated via the Notification of Changes for Business Entity Form link.
The reviewing/changes will need to be completed by an individual affiliated with the business under one of the following designation types**:**
- Director
- Licensing Coordinator
- Manager
- Member
- Officer
- Owner
- Partner
- President
- RDA
- Secretary
- Treasurer
- Vice President
