Posted August 1, 2025, The Ohio Department of Insurance sent out a reminder that all Business Entities must review and update their affiliations in advance. Affiliations can be reviewed and updated via the Notification of Changes for Business Entity Form

The reviewing/changes will need to be completed by an individual affiliated with the business under one of the following designation types**:**
- Director
- Licensing Coordinator
- Manager
- Member
- Officer
- Owner
- Partner
- President
- RDA
- Secretary
- Treasurer
- Vice President
